The Trenton Children’s Health Project (TCHP), a partnership between Henry J. Austin Health Center (HJAHC) and Children’s Health Fund (CHF), was launched in 2019 to further support the need for high quality healthcare for communities in underserved areas of greater Trenton, New Jersey. HJAHC is the leading healthcare center in Mercer County and reaches about one-third of the population of Trenton, one-third of which are children. By integrating primary care, mental health, and substance abuse services, TCHP improves healthcare outcomes and effectively serves patients with multiple needs.

Serving Communities in Need: TCHP serves all Mercer County residents with or without medical insurance with comprehensive, quality, and patient-centered primary care, pediatrics, women’s health, behavioral health, dental, vision, pharmacy and podiatry services. Over 16,000 patients access facilities in eight Trenton locations. Two-thirds of patients served are at or below the poverty line, 60% are on Medicaid, and 10% are homeless. Over 50% of the patient population are African-American and female, and one-third are Latino. Given the complex nature of patient needs, TCHP delivers services far beyond simple primary care. Clinical pharmacists are embedded within healthcare teams to provide recommendations on appropriate therapy and integrated care sites ensure people within mental health facilities have improved access to primary care. TCHP is continually using new and innovative technologies to manage and improve patient care, including using telemedicine to access top specialists from across the nation. With an advanced access system, every established patient can be seen within two days. TCHP plans on launching a new Mobile Health Unit program partnering with our community school system to reach more children in more communities, starting with high-risk schools where children have inadequate access to primary care services.
