New York Programs

The New York Programs are a network of Federally Qualified Health Centers that are committed to providing comprehensive, coordinated and compassionate patient-centered care to all persons regardless of ability to pay, or immigration status. The NY Programs, including the New York Children’s Health Project, the South Bronx Health Center, Center for Child Health and Resiliency and the Center for Preventive Health and Special Initiatives, serve as a patient-centered medical home providing quality care through a variety of integrated services at its mobile and primary care clinics. Since its inception, the NY Programs have been increasing access to care, combating health disparities and training health professionals to work with the most high-risk, underserved, and underrepresented populations.

The South Bronx Health Center (SBHC) and its expansion site, the Center for Child Health and Resiliency (CCHR), both provide comprehensive care to children and families living in the South Bronx, one of the nation’s poorest congressional districts. SBHC serves a community where there were few existing comprehensive health services before CHF’s facility was established in 1993. Skilled and sensitive medical professionals in these health centers provide quality care for 8,000 children and adults annually.

The Center for Preventive Health and Special Initiatives is the program evaluation and research unit of the NY Programs and focuses on treating childhood asthma and obesity, diabetes, mental health conditions, and other key health disparities experienced by low-income populations. The Center also leads quality improvement and evaluation efforts for the NY Programs.
